
Immunization Requirements
Students entering their first year of school:
(Kindergarten or First Grade)

Hepatitis B: 3 doses |

DTP/DtaP/DT: 5 or more doses, one of which must be given after the fourth birthday. If starting the series at age seven or older, only three doses are required. |

Polio (OPV/IPV): 4 doses of OPV or IPV. If the 3rd dose was given on or after the fourth birthday, the 4th dose is not required. However, if a combination of IPV/OPV (2IPV/2OPV) is used, all 4 doses are needed regardless of age. |

Measles, Mumps, Rubella (MMR): 2 doses, given on or after the first birthday (The second dose should be given at least 30 days after the first.) |

1) Hepatitis B: 3 doses given over a six month period.The Morgan County Health Department is providing the Hepatitis B vaccine free of charge for students in the 6th grade.
Students entering the 8th grade:
1) Proof that the second MMR immunization was received
Students entering the 12th grade:
1) Proof that the second MMR immunization was received
